Once your cordon (vining) tomato reaches the top of its string, you can try the same technique that commercial growers use - use the slack in the string to lower the stem down so that the bottom truss is just off the ground, then there is room for another 30cm or so of upward growth. By the time the plant reaches the top of the string again, the bottom truss will be ripe and picked, and you can lay the stem down again.
